Altitude, clear air, and no clouds.

Three factors that each sound minor and compound into something that is not. Skin that has lived twenty years in this valley has a measurably different history from skin that has lived twenty years anywhere humid, and almost everything we treat here traces back to it.

Altitude

Tucson sits at about two thousand four hundred feet, and ultraviolet intensity rises with elevation because there is less atmosphere above you to absorb it. It is a real effect and it is why mountain sunburn happens faster than people expect.

Clear air

Low humidity and little particulate means less scattering and less absorption on the way down. The same sun at the same angle simply delivers more here than it does through humid coastal air.

Dryness

Why the pigment comes first

The most common thing that walks in here is not lines, it is uneven brown pigment across the cheeks and upper lip: sun-induced melasma and solar lentigines, arriving ten years earlier than the textbooks describe.

Melanocytes respond to ultraviolet by producing melanin, and repeated stimulation over decades makes some of them permanently overactive. That is why sun damage keeps appearing for years after the exposure that caused it, and why people are surprised to develop pigment in their forties from a childhood spent outside.

It is also why light-based treatment here is seasonal. Treating melanocytes that are being actively stimulated is treating a moving target, and the pigment comes straight back.

The four things that actually help

Sunscreen, daily, including winter and including indoors. Broad spectrum, SPF 30 minimum, reapplied if you are out. This is genuinely the highest-impact item on this entire website and it costs about thirty dollars a bottle. UVA passes through window glass and through a car windscreen, which is why the left side of most drivers’ faces in this country is measurably more aged than the right.

A hat with a real brim. Three inches or more, all the way round. A baseball cap does nothing for cheeks, which is where the pigment is.

A retinoid at night. The only topical class with strong evidence for reversing photodamage rather than just preventing more.

Barrier repair. Ceramides and a moisturiser that actually occludes, particularly in the driest months. It is unglamorous and it is why our skin here reacts to treatments that people tolerate easily elsewhere.

The windscreen point, properly

Car windscreens are laminated and block most UVA. Side windows are usually not laminated and block much less. Anyone who commutes with their left arm and left cheek in the sun is accumulating asymmetric damage, and over decades it is visible enough that dermatology journals have published photographs of it.

In a city where people drive a lot in relentless sun, this is not a curiosity. It is a meaningful proportion of the pigment we treat, and it is solved by sunscreen in the morning rather than by anything that happens here.

What we cannot undo

Worth saying plainly, because this industry rarely does. Laser and light treatment can clear existing pigment and stimulate collagen, and it is genuinely effective. It does not reverse the underlying photoaging of the dermis, and it does not stop the melanocytes that are already overactive from producing again.

Which is why every plan that leaves this building has sunscreen at the top of it, and why we would rather treat somebody once and have them protect the result than treat them every year forever. The second version is better business and it is not what anybody here wants to do.

Is the sun stronger in Tucson?
